What Information We Collect

TeleMed2U only collects other personal information about you through the Services only when you voluntarily provide it to us. If you choose to go through our screening process and/or register for the Services we ask you to provide personal information, including but not limited to: your name, address, telephone number and/or email address, height, weight, and health information in connection with the use of our products or services. We also collect information that will allow you to establish a username and password. You may choose not to provide us with certain information, but that may result in our inability to provide you access to the Services.

What Information We Disclose

By using the Services you consent to and authorize TeleMed2U and its affiliates to disclose your eligibility for and participation in the Services (i.e. that you meet the enrollment criteria for the Services and that you have elected at your own discretion to participate).In particular, TeleMed2U and any of its affiliates may be able to access the information you provide to us for the purpose of delivering the care requested by you.

No mobile information will be shared with third parties/affiliates for marketing/promotional purposes; all the above categories exclude text messaging originator opt-in data and consent; this information will not be shared with any third parties. How We Use Personal Information That We Collect Online

(a) Internal Uses - We may use information we collect about you to:

  • Provide you with access to particular tools and services;
  • Respond to your inquiries and send you administrative communications;
  • Obtain your feedback on our websites and our offerings;
  • Statistically analyze user behavior and activity;
  • Provide you with more relevant content;
  • Conduct research and measurement activities;

We may combine personal and non-personal information collected by TeleMed2U about you, and may combine this information with information provided by external sources.  

We limit TeleMed2U’s employees’ and contractors’ access to personal information. Only those employees and contractors with a business reason to know have access to this information, and then may only access or use the minimum necessary for the task at hand. We educate our employees about the importance of maintaining confidentiality of user information.

(b) Special Circumstances
The following categories describe unique circumstances in which TeleMed2U may use or disclose your PHI:

  1. Public Health Activities. TeleMed2U may disclose your PHI to public health authorities or other governmental authorities for purposes including preventing and controlling disease, reporting child abuse or neglect, reporting domestic violence and reporting to the Food and Drug Administration regarding the quality, safety and effectiveness of a regulated product or activity. TeleMed2U may, in certain circumstances disclose PHI to persons who have been exposed to a communicable disease or may otherwise be at risk of contracting or spreading a disease or condition.
  2. Health Oversight Activities. TeleMed2U may disclose your PHI to a health oversight agency for authorized activities such as audits, investigations, inspections, licensing and disciplinary actions relating to the healthcare system or government benefit programs.
  3. Judicial and Administrative Proceedings. TeleMed2U may disclose your PHI, in certain circumstances, as permitted by applicable law, in response to an order from a court or administrative agency, or in response to a subpoena or discovery request.
  4. Law Enforcement. TeleMed2U may, under certain circumstances, disclose your PHI to a law enforcement official, such as for purposes of identifying or locating a suspect, fugitive, material witness or missing person.
  5. Decedents. TeleMed2U may, under certain circumstances, disclose PHI to coroners, medical examiners and funeral directors for purposes such as identification, determining the cause of death and fulfilling duties relating to decedents.
  6. Organ Procurement. TeleMed2U may, under certain circumstances, use or disclose PHI for the purposes of organ donation and transplantation.
  7. Threat to Health or Safety. TeleMed2U may, under certain circumstances, use or disclose PHI if necessary to prevent or lessen a serious and imminent threat to the health or safety of a person or the public.

Cookie Placement

Certain TeleMed2U websites, like many other commercial websites, may use a standard technology called “cookies” to collect information about how our website is used. Cookies were designed to help a website operator determine that a particular user had visited the site previously and thus save and remember any preferences that may have been set.
