Here’s What You Need to Know

Prior to your upcoming appointment

1. Register for your Advanced MD Patient Portal!
  • You will receive an email with instructions to create your patient portal account. Use your email and create a password
  • If you did not receive an email with instructions, please click the link to self-register HERE.
  • You will then be logged into your portal! Where you can do things like schedule an appointment, access your medical records, request prescription refills and more.

2. Complete Intake Paperwork BEFORE your Appointment
As part of the transition, you’ll receive an email to complete your intake forms ahead of your next visit.

Completing these forms with up-to-date information is essential to ensure a smooth appointment and allows us to provide the most effective care.
3. Fill Out Updated Consent Forms
With the launch of AdvancedMD, all patients are required to complete new digital consent forms.

These forms cover important updates to our policies and MUST be signed before your next appointment. You’ll receive the consent form alongside your intake paperwork in your portal.
