Fill Out Updated Consent Forms
With the launch of AdvancedMD, all patients are required to complete new digital consent forms. These forms cover important updates to our policies and must be signed before your next appointment. You’ll receive them alongside your intake paperwork.
Klara Will No Longer Be Our Primary Communication Tool
With the launch of our new system; Klara will no longer be used for patient communication.
