Program Summary

What is the Diabetes Care Program?

With TeleMed2U Diabetes Care Program you will have virtual visits with both
a) Endocrinologist or expertly trained Physician Assistants
b) Clinical Diabetes specialist who will provide additional support, health  education and advice to help you succeed.

For your first visit, you will be scheduled with an Endocrinologist and a clinical diabetes specialist. Similar to an office-based visit, you may receive prescription medications and lab orders as well as an order for a continuous glucose monitor. After the initial visit you will be scheduled for all subsequent visits for the rest of the year. In between scheduled visits, you will have seamless access to communicate with your care team. We may recommend medication or lifestyle adjustments.

What will be included in the program?

An individualized treatment plan designed and tailored around your personal needs.
Labs and prescriptions will be ordered by your care team as needed.
Remote monitoring of blood sugars and text based guidance on how to improve in between visits.

Who can enroll in this program?

Anyone who has Type 1 or Type 2 diabetes and resides in the following states  and  wants a team of healthcare professionals invested in achieving better outcomes for them. This plan is also suitable for people who do not have insurance as our mission is to make Specialty care more accessible and affordable across the US.

What will be included in the membership?

You will get access to a multifaceted clinical team made up of Certified Clinical Diabetes Specialists, Endocrinologists and an entire clinical team invested in your success. The program includes visits with these providers, ongoing monitoring and support to achieve the best outcome for you based on your personalized treatment plan.

Do I have to use CGM to participate in this program?

No, however while we strongly encourage the use of continuous glucose monitoring (CGM) as it allows us to monitor your glucose levels remotely. We can still help you with your diabetes using standard finger stick blood sugars.

How much does CGM cost if I don't have insurance?

The cash prices for sensors vary, for example a Freestyle Libre sensor is usually between $70 to $80 per sensor. You would need 2 sensors per month so expect a cost between $140 to $160. Please note these prices are based on checking major outlets like Walmart and Walgreens, we cannot guarantee these prices.

Can the doctor prescribe medication and how do I receive my prescription?

Yes, TeleMed2U doctors can prescribe medications like any other doctor you visit.

Our doctors will do an initial evaluation to build out your personalized treatment plan and include prescription medication when appropriate to help achieve the best outcomes for you.. The prescriptions are sent electronically to the pharmacy of your choice.

Who do I call if I need to cancel or reschedule my specialty visit?

Is my consultation private and secure?

Yes, TeleMed2U’s platform is HIPAA and HITECH compliant. 

All interactions between you and your doctor are encrypted and remain strictly confidential.

Do I need a referral from my primary care physician to enroll?

No, You do not need a referral from your PCP to enroll in our Diabetes Care Program. If your insurance company requires a referral, our staff will work with you to obtain the required referral.

What if I have a question for my diabetes care team after my appointment? What is the expected response time?

Yes, You have the ability to send a message to us 24 hours a day, 7 days a week; however, responses to messages are typically sent Monday through Friday, 9am to 5pm (Pacific Time). Questions asked after hours will usually be answered the following business day.
